Building Compassionate Communities is a pilot project that has developed at a community level which will: • Ensure communities have the skills, capacity and appropriate networks to support each other before and after End of Life, address the social stigma around talking about End of Life and improve death literacy. • The outcomes sought are: • Good End of Life Care supported by a compassionate community, more people better prepared to discuss, make informed choices and plan end of life preferences (before and after death), more people dying in preferred place of death, carers that are supported to remain resilient and continue to play a key role in helping people to remain at home and independent longer.
